Shift lever position?

Alright, the manual says the shifter lever should be even with the footpeg. I know it is personal preference. but how do you have your shifter lever positioned?

Is it above your toe? under your toe? or at the end of your toe? under normal riding?

I saw this on a checklist I found. Does this sound about right?

"Ride around a bit, make sure your shift lever is adjusted correctly. When you move your toe under the shift lever, you should have to move your toes downwards just a little bit to get it underneath. Your foot should be in contact with the shift lever as soon as you have your foot under it - if you have to pull up at all to touch your shift lever, it's too high. When adjusted correctly, the shift lever is about at a level with the center of your toes when your feet are in their relaxed position."

I like to ride with the balls of my feet on the foot pegs, and my knees against the tank. I'll move my feet forward just before I have to shift or brake. I've gone around a tight left hander with my foot under the shifter. Big mistake, as my foot was spit back. Hurt my foot and upset the bike's balance.

(edit) My CBR has a dimple on the shift lever that is lined up with a dimple on the shaft. That's the stock position. You can take the lever off and move it up or down a notch to try different positions. (edit)

 So when I get into a wheel stand my right foot wants to rotate forward on the peg and almost forces my foot to push the rear brake.
I also have a problem when I stand on my pegs my gear shifter is down just a bit too low and wants to up shift.
I can get the shifter worked out but this brake thing has got me bugged.
Thanks for any help you guys can give me.
Also I was wondering what the GP style shifting is and how does it work?
Lost in this pond.
